Prairie Mountain Health region released a schedule for June pop-up clinics for the Pfizer and Moderna vaccines.
The listed clinics are for first and second dose eligible Manitobans. Pfizer clinics first dose eligibility is 12 years and older. The Moderna clinic’s first dose eligibility is 18 years and older. The criteria for the second dose changes frequently. Please visit protectmb.ca for the most up-to-date information.
The following locations are where immunizations for the Pfizer and Moderna vaccines will be available:
DAUPHIN
• Pfizer – June 1, 2, 6, 7 ,8, 9, 13, 14, 20, 21, 22, 27 – Ukrainian Orthodox Hall, 204 Whitmore Ave. E, 11 a.m. to 6 p.m.
• Moderna: June 15, 16, 23, 28, 29, 30 – Ukrainian Orthodox Hall, 204 Whitmore Ave. E, 11 a.m. to 6 p.m.
DELORAINE
• Pfizer – June 3 & 27 – Deloraine Winchester Hall, 119 Lake St. N, 11 a.m. to 6 p.m.
• Moderna – June 25 – Deloraine Winchester Hall, 119 Lake St. N, 11 a.m. to 6 p.m.
KILLARNEY
• Pfizer – June 1 & 29 – Shamrock Center, 725 Broadway Ave, 11 a.m. to 6 p.m.
• Moderna: June 16 – Shamrock Center, 725 Broadway Ave, 11 a.m. to 6 p.m.
NEEPAWA

VIRDEN
• Pfizer – June 9 & 28 – Tundra Oil and Gas Place, 900 5th Ave. N, 11 a.m. to 6 p.m.
• Moderna – June 23 -Tundra Oil and Gas Place, 900 5th Ave. N, 11 a.m. to 6 p.m.
For people 12 to 15 years old, who are attending a Pfizer vaccine clinic, Prairie Mountain Health is asking that a parent, guardian or caregiver attend the appointment with them or bring a signed consent form at the time of their appointment. If the youth attends without a guardian and without a signed consent form, they will go through an informed consent process with a clinical lead to assess their ability to consent on their own and proceed with the vaccine. Young people aged 16 and 17 can sign their own consent form.
Also, people are reminded to bring their MB Health Card and a piece of identification.
If you meet the eligibility criteria, call 1-844-626-8222 to book your vaccine appointment. Dauphin or Brandon super sites can only book online at protectmb.ca/ or call 1-844-626-8222.
Individuals can book for other people but require that person’s Manitoba Health card number.
Second dose vaccine appointments can also be booked if you meet the following criteria and the minimum time between doses.
The current eligibility criteria as of May 26 is:
• You received your first dose March 29 or earlier
• All Indigenous people ages 12 and up
• People with priority health conditions (i.e. receiving hemodialysis, liver cirrhosis, severe heart failure, received or to receive solid organ transplant, receiving chemotherapy etc.)

To book a second dose appointment, people must have:
• received a Moderna vaccine 28 days before their second dose appointment date
• received a Pfizer vaccine 21 days before their second dose appointment date
Prairie Mountain Health stressed the importance of bringing a completed consent form to your vaccine appointment. A consent form is required for both doses.

Copies of consent forms are also available at PMH Health Care facilities, PMH community town and RM offices for pick up.
